WAX OF THE ELITE provides high-quality tables rated up to 400 lbs. Please inform your esthetician of any accommodations needed for your safety or comfort. We will happily adjust to meet your needs.

We do not discriminate on the basis of weight or physical ability.

WAX OF THE ELITE is not liable for injuries related to equipment use if accommodations were not disclosed.

INFORMED CONSENT FOR SERVICES

I understand that services offered may involve removal of hair or treatment of skin using waxing, sugaring, tinting, lamination, and lightening techniques.

I acknowledge that I have disclosed all relevant medical and skin information to my esthetician.

I understand that the following side effects may occur: redness, irritation, swelling, bumps, welts, bruising, hives, scarring, hyperpigmentation, allergic reactions, and skin lifting.

I understand these side effects are usually temporary and mild but can occasionally be more serious.

I have had the opportunity to ask questions and understand the procedures, including risks and aftercare.

I understand that I should avoid sun exposure, retinoids, exfoliants, and tanning before and after treatments.

LIABILITY WAIVER & RELEASE

I hereby release WAX OF THE ELITE LLC, its owners, affiliates, staff, and authorized personnel from any and all liability for any injury, condition, or side effect resulting from my participation in full body waxing / sugaring, skin lightening, brow services, or any procedure performed.

I understand that results may vary and no specific outcome is guaranteed.

I waive all present and future claims and agree not to hold WAX OF THE ELITE LLC responsible for reactions due to undisclosed health information, failure to follow aftercare, or any unforeseeable condition.

I acknowledge that electronic records and signatures are valid and legally binding.

By signing below, I confirm that the information I have provided is accurate and complete to the best of my knowledge.
